October 2012 – Care Coordination
On October 18, 2012, The Shriver Center presented a webinar for providers on Care Coordination. Attendees learned more about what Care Coordination is and what it means for them and their clients. It also included information on the Care Coordination Innovations Project, the impact of these projects on health, housing, and human service providers, and what impact these projects will have on the Illinois Medicaid program’s long-term fiscal health.
